REQUIRED FORM
An Activity/School Trip Permission/Emergency/Code of Conduct Form must be submitted for every student. This form covers various on and off-campus, during, and after-school activities for all grades. Your student will not be able to attend any of these events without its completion.
Parent/Guardian 1 and Parent/Guardian 2 will receive this form VIA EMAIL on Monday, August 3, 2026.
Confirm that you have completed the form before continuing.
If not, go to your inbox and look for the subject line "Form for Lastname Firstname titled SRVHS Activity/School Trip Permission/Emergency/Code of Conduct Form 2026-27" sent by forms@informedk12.com. Only one submission per student is required (either Parent/Guardian 1 or Parent/Guardian 2 may submit the form).
